Routine nail trimming and filing, skin observation, moisturizing, review of concerns and updates to your established foot-care plan.

Initial Foot Care Assessment & Treatment

A first visit including health history, foot and skin assessment, nail care as appropriate, personalized foot-care guidance and a care plan. Additional referral may be recommended when needed.

Home Support Services

Personalized non-clinical assistance to support independence at home, including meal preparation, light housekeeping, laundry, companionship, errands and help with daily routines.

Initial Nursing Home Visit

For separately assessed nursing services that are within your authorized LPN scope and demonstrated competence, with a clear care plan and appropriate insurance coverage.
